We provide billing documentation that insurance companies require: patient data,
provider data, procedure codes, and diagnosis codes, etc. We can provide you with a Super Bill, or we will submit HCFA 1500 forms
to your insurance company.
Payment of Fees
Unless prior arrangements have been made (e.g. in PIP auto accident
cases), all fees are payable at the time of service.
